POSITION DESCRIPTIONS
Licensed Professional Counselor
The Licensed Professional Counselor will offer guidance to consumers, couples, families and groups who are dealing with issues that affect their mental health and well-being. The LPC will work holistically, using a “wellness” model (as opposed to an “illness” model) which highlights and encourages client’s strengths.
Responsibilities include but are not limited to:

Work with consumers as individuals, groups and communities to improve mental health.

Encourage consumers to discuss emotions and experiences.
-
Examine issues including substance abuse, aging, bullying, anger management, careers, depression, relationships, LGBTQ issues, self-image, stress and suicide.
-
Work with families.
-
Help consumers define goals, plan action and gain insight.
-
Develop therapeutic processes.
-
Refer clients to psychologists and other services.
-
Take a holistic (mind and body) approach to mental health care.Must be a graduate student in good standing in an accredited university.
Basic Qualifications:
-
Must have an active LPC license in the Washington, D.C.
-
Must have a recent background check including a DC Police clearance. VERY IMPORTANT: If you reside in Maryland or Virginia you will also need an additional Police clearance from that state.

Proof of Limited Liability Insurance.

A TB test no older than six months from the date of the application.

Must be fully capable of using and mastering technology required to perform the job. (There will be NO EXCEPTIONS TO THIS RULE)
Community Support Worker
Community Support Worker's play an integral role in the success of PG Behavioral Health Services providing vital resources to consumers who live with mental illness. Through outreach and collaboration the Community Support Worker will follow the treatment plan and help the address the consumers individual needs.
Responsibilities include but are not limited to:
-
Aids consumers in their own homes/program and in the community to allow them to function more safely and independently. Facilitates physical, recreational, social and educational activities. Provides consumers with a positive role model.

Using technology and resources provided by P&G Behavioral Health Services determines the best social, economic, recreational and educational services in the community that will meet consumer’s needs; Maintains liaison with other agencies, professionals, government officials and the community.

Recognizes potential crisis situations, analyzes such situations accurately, develops strategies to deal with such situations, and informs the Team Leader and supervisor when such incidents arise.

Assists case management team in identifying potential problems. Provides input to counselors and other professionals with regard to the development of consumer program plans.

Monitors consumer’s well-being. Assists consumers with daily life skills and/or behavior management training. Assists consumers with basic cooking, etc. Assists consumers in attending appointments when necessary and may provide transportation.

Monitors consumers’ progress and well-being and assists case management team in evaluating the effectiveness of planned counseling and support. Provides suggestions for the modification of consumers’ program plans.
-
Encourages and facilitates the participation of individuals in the program. Creates a fun and enjoyable atmosphere in order to enhance the program’s appeal to consumers.
Basic Qualifications:
-
50% or more of the work takes place in the field at the consumers choice of location within their own communities. To perform the full duties of the job the employee is required to use hands to finger, handle, or feel and reach with hands and arms. The employee is occasionally required to stand, walk, sit and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move 20+ lbs., and/or move to 35+ lbs.

Bachelor's degree with three (3) years in case management role preferred; degree and experience may be substituted with an equivalent combination of skills and related experience.
Mental Health Case Manager
The role of the case manager is to undertake assessment, monitoring, planning, advocacy and linking of the consumer with rehabilitation and support services by coordinating the overall plan of care for patients with mild to moderate emotional and behavioral problems. The case manager will oversee consumer evaluation and treatment planning, and monitors consumer progress, adjusting treatment plans as necessary.
Responsibilities:
-
Participates as directed in the screening of new service requests within the clinic by means of and agency approved procedure which ensures that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing Board policies and procedures.

Participates in regular interdisciplinary staff meetings held at position location.
-
Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessment on all clients entering caseload.

Develops appropriate treatment/service plans with clients as the means for implementing appropriate services and developing effective alliances with clients.
-
Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service plan in order to help the clients to achieve the stated goals and objectives.
Qualifications:
-
Demonstrated ability to interview and assess clients, using appropriate assessment tools, and observe, record and report on an individual’s functioning; to read and understand assessments, evaluations, observation, and use in developing treatment plan; to identify community resources and services for clients and coordinate provision of services; to establish effective working relationships with internal agency staff as well as with relevant community organizations; interact positively with consumers and their families, work as a team member.

Communicate effectively, verbally and in writing, to maintain confidentiality, and to work independently under general supervision.
-
Must possess a combination of mental health work experience or relevant education, and training which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties.
-
Bachelor degree in relevant human services field plus at least one year experience to meet Qualified Mental Health Professional according to DBH requirements preferred.
